ELIGIBILITY:
Effective first day of the month following 90 days of regular, full-time employment.
COST:
Associate’s coverage provided at no cost to associate at the Value PPO level.

401(K) PLAN
ELIGIBILITY:
Effective the first month of the quarter following 90 days of regular, full
time employment. Minimum age 21 years.
ASSOCIATE CONTRIBUTIONS:
Between 1% & 100% of associate’s pay not to exceed government limits.
EMPLOYER MATCH:
Employer matches 50% on the first 6% of associate contributions.
VESTING:
For company contributions: 50% after 2 years, 75% after 3 years, and 100% after
4 years.
INCENTIVE PLAN
ELIGIBILITY:
After 6 months of continuous regular, full time employment, vacation accrues on
employment anniversary date.
POTENTIAL:
The potential incentive plan bonus amount is based on annual performance on individual
and company performance goals, salary grade, and full months of service during the
plan year.
VACATION PLAN
ELIGIBILITY:
After 6 months of continuous regular, full time employment, vacation accrues on
employment anniversary date.
TIME EARNED:
5 days for 6 months of service; 5 days for 1 year of service; 2 weeks for 2-4 years
of service: 3 weeks for 5-8 years of service; 4 weeks for 8 or more years of service.
Vacation time may not be carried over into the succeeding year based on anniversary
date.
SICK LEAVE
ELIGIBILITY:
Associates with three full calendar months of regular, full-time employment.
BENEFIT:
Earned at the rate of 4 hours for each full month of employment. The maximum sick
time that may be carried over into the next year is 6 days (48 hours). Each
December 31st, if an associate’s sick leave bank has more than 10 days (80 hours)
of accrued time but unused sick time, the remaining accrued but unused balance will
be transferred to a Supplemental Sick Time account for the associate. Supplemental
sick time is restricted sick time for use in any medical situation allowed under
the Family and Medical Leave Act.
PERSONAL HOLIDAY
ELIGIBILITY:
One Personal Holiday will be granted after six months of regular full-time employment
to be used in the current calendar year.
BENEFIT:
Associates earn two “floating” personal holidays per calendar year that must be
used during that year.
HOLIDAYS
ELIGIBILITY:
Immediately upon regular, full time employment.
EXCEPTION:
Depending upon business needs, some departments may be required to work on selected
holidays, but associates will receive Holiday Pay benefits.

PARKING
ELIGIBILITY:
Immediately upon employment.
BENEFIT:
Parking in a surface lot is provided at no cost to associate.
HEALTH CLUB REIMBURSEMENT
ELIGIBILITY:
Immediately upon regular, full time employment.
BENEFIT:
Associates will be reimbursed for 50% of the monthly cost of membership dues to
a Health Club or nutritional program up to $20 per month. The associate must provide
the expense report within 30 days of the month for which the bill is provided.
CAFETERIA PLAN
ELIGIBILITY:
Effective first day of the month following 90 days of regular, full-time employment.
DEFERRALS:
An associate may elect to have a pre-tax payroll deduction deferred into a Flexible
Spending Accounts up to an annual maximum of $1,500 for the Health Care Expenses
and $2,500 or $5,000 for the Dependent Care Expenses
EDUCATIONAL ASSISTANCE
ELIGIBILITY:
90 days of regular, full-time employment.
BENEFIT:
Associates are eligible for reimbursement of job-related training or college courses
based on their final course grade at public institutions’ tuition rates. Courses
subject to company pre-approval.
